If you are interested in playing an intercollegiate sport at Framingham State this year, there are several forms you need to fill out before you may begin practicing. This year all of the forms – with the exception of the medical forms (physical, medical history, proof of insurance) – are being done electronically. You have direct access to the athletic training forms through the Athletic Training section of the Athletics website, but in order to access the NCAA-mandated forms, you need to fill this short form out so that you can be activated in the NCAA system. Once you have been activated, you will receive an email with a link to the NCAA forms. Please note that you can not fill out the NCAA forms until you have been registered in the system as each student-athlete is assigned a different code. Forwarding the email to anyone who hasn’t been registered in the system will not work for those recipients. For those student-athletes wishing to play a fall sport (men’s soccer, women’s soccer, field hockey, football, men's cross country, women's cross country and women’s volleyball), these forms need to be completed before you move in for pre-season camp. For those student-athletes wishing to play a spring sport whose non-traditional season is in the fall (baseball, softball, women’s lacrosse), these forms need to be completed by September 1. Winter student-athletes (men’s basketball, women’s basketball, and ice hockey) should have these forms completed by the first day of classes.
